About the Atal Amrit Abhiyan Health Insurance Scheme

• Atal Amrit Abhiyan is a health insurance scheme launched by the
  Government of Assam.
• Under the scheme, the state government provides
  health insurance coverage of up to ₹2 lakh per family per year.
• The scheme was originally launched on 25 December 2016 and is
  currently operational.
• The following benefit is provided under the
  Atal Amrit Abhiyan Health Insurance Scheme:
  • Health insurance coverage of up to ₹2 lakh per year.
• The following persons are eligible under the scheme:
  • The applicant must be a resident of Assam.
  • BPL families with an annual income of less than ₹1.20 lakh.
  • APL families with an annual income between
    ₹1.20 lakh and ₹5 lakh.
  • Serving or retired Central and State Government employees
    are not eligible under the scheme.
• The Atal Amrit Abhiyan application form can be downloaded in
  PDF format from the official website.
• The scheme is designed for BPL and APL families suffering from
  chronic, sudden, or acute health conditions.
• Beneficiaries belonging to the APL category are required to pay a
  nominal premium of ₹100 per year per member.
• Eligible persons can avail the benefits of the scheme by applying through the offline application form

Eligibility criteria for Atal Amrit Abhiyan Health Insurance Scheme

• The following persons are eligible under the
  Atal Amrit Abhiyan Health Insurance Scheme:
  • The applicant must be a resident of Assam.
  • BPL families with an annual income of less than
    ₹1.20 lakh.
  • APL families with an annual income between
    ₹1.20 lakh and ₹5 lakh.

• The following documents are required at the time of registration under the scheme:
  • Voter ID card
  • EPIC card (for minors, original birth certificate is required)
  • Income certificate
  • NFSA card (if available)
  • Recent photograph of the patient, attested by a registered doctor

Application Process of Atal Amrit Abhiyan Health Insurance Scheme

1. The Atal Amrit Abhiyan Application Form can be downloaded in PDF format from here
2. Fill in all the required basic details in the application form.
3. Attach all the required supporting documents.
4. Submit the duly filled application form to the concerned authority.
5. The submitted application will be verified by the designated officer.
6. After approval, the benefit amount will be transferred to the applicant’s bank account.
