About the Atal Amrit Abhiyan Health Insurance Scheme
- Atal Amrit Abhiyan is a health insurance scheme launched by the
Government of Assam. - Under the scheme, the state government provides
health insurance coverage of up to ₹2 lakh per family per year. - The scheme was originally launched on 25 December 2016 and is
currently operational. - The following benefit is provided under the
Atal Amrit Abhiyan Health Insurance Scheme:- Health insurance coverage of up to ₹2 lakh per year.
- The following persons are eligible under the scheme:
- The applicant must be a resident of Assam.
- BPL families with an annual income of less than ₹1.20 lakh.
- APL families with an annual income between
₹1.20 lakh and ₹5 lakh. - Serving or retired Central and State Government employees
are not eligible under the scheme.
- The Atal Amrit Abhiyan application form can be downloaded in
PDF format from the official website. - The scheme is designed for BPL and APL families suffering from
chronic, sudden, or acute health conditions. - Beneficiaries belonging to the APL category are required to pay a
nominal premium of ₹100 per year per member. - Eligible persons can avail the benefits of the scheme by applying through the offline application form

Eligibility criteria for Atal Amrit Abhiyan Health Insurance Scheme
- The following persons are eligible under the
Atal Amrit Abhiyan Health Insurance Scheme:- The applicant must be a resident of Assam.
- BPL families with an annual income of less than
₹1.20 lakh. - APL families with an annual income between
₹1.20 lakh and ₹5 lakh. - Serving or retired Central and State Government employees

- The following documents are required at the time of registration under the scheme:
- Voter ID card
- EPIC card (for minors, original birth certificate is required)
- Income certificate
- NFSA card (if available)
- Recent photograph of the patient, attested by a registered doctor
