Information about Assam Chief Minister Free Diagnostic Services Programm

The Chief Minister Free Diagnostic Services Programme is a healthcare initiative of the Government of Assam for the people of the state.

• The programme was launched on 11th May 2017.
• The primary objective of the scheme is to provide free and quality diagnostic services to the residents of Assam.
• The Department of Health & Family Welfare, Assam is the nodal department for implementation of the programme.
• Under this programme, the Government of Assam provides the following diagnostic services free of cost:
  • CT Scan services
  • X-Ray services
  • Laboratory services including blood tests and imaging investigations
• No charges are levied from beneficiaries, irrespective of their income status.
• All residents of Assam, whether APL or BPL, are eligible to avail services under the scheme.
• More than 56 laboratory tests are covered under the programme.
• Diagnostic services are provided through HLL Lifecare Limited, a Government of India enterprise.
• Till date, more than 64 lakh patients have availed free diagnostic services under this programme.
• To avail the services, beneficiaries must visit a Government Hospital or Authorized Diagnostic Centre with any one of the following identity proofs:
  • Domicile of Assam
  • Voter ID card
  • Aadhaar card
  • Driving licence

The following benefits are provided under the Assam Chief Minister Free Diagnostic Services Programme:

• Free of cost CT Scan services
• Free of cost X-Ray services
• Free of cost laboratory services, including blood tests and imaging scans

Eligibility Conditions

• The beneficiary must be a permanent resident of Assam.
• Beneficiaries of all age groups are eligible.

Documents Required for Assam Chief Minister Free Diagnostic Services Programm

• Any one proof of residence:
  • Domicile of Assam
  • Voter ID card
  • Aadhaar card
  • Driving licence
• Medical prescription issued by a doctor specifying the required diagnostic test

How to Apply for Assam Chief Minister Free Diagnostic Services Programm

• Beneficiaries do not need to apply separately to avail the benefits of the Assam Chief Minister Free Diagnostic Services Programme.
• Beneficiaries can directly visit any of the following facilities, wherever services are available, to avail free CT Scan, X-Ray, and laboratory services:
  • District Hospitals
  • Government-aided hospitals
  • Government-empanelled hospitals
  • Public Health Centres (PHCs)
  • Community Health Centres (CHCs)
  • Authorized diagnostic centres
• At the facility, beneficiaries are required to fill the service request form and attach the necessary documents.
• After initial verification, the prescribed diagnostic test is conducted free of cost, and no fee is charged from the patient.
